4 Key Barcode Types and Their Standards Simplified

In the medical device industry, compliance with barcode standards is essential for device identification and traceability—especially under UDI (Unique Device Identification) systems. Moreover, these standards play a critical role in patient safety. Bar Code Types and Characteristics

1. Health Industry Bar Code (HIBC)

Purpose:
The Health Industry Business Communications Council (HIBCC) developed the Health Industry Bar Code (HIBC), a barcode standard specifically tailored for the healthcare industry. It supports FDA UDI compliance by encoding key device information — such as quantity, lot number, serial number, and expiration date—for product identification and tracking. HIBC barcodes not only enhance communication between manufacturers and healthcare providers, but they also ensure efficient and accurate item traceability.

Format:
Linear (Code 128) or 2D (DataMatrix).

Data Structure:
Starts with a “+” symbol (primary data structure), and includes labeler ID, product catalog number, and unit of measure.  
Secondary structure contains batch/lot number, expiration date, serial number.

Usage:
Users can scan HIBC barcodes using standard barcode scanners and readers, which enables quick and accurate data retrieval. In addition, this system provides a structured method to identify and track medical devices, supplies, and pharmaceuticals. As a result, manufacturers and hospitals frequently rely on HIBC barcodes for inventory management and product tracking of devices, implants, and surgical instruments.

Regulatory Role:
The FDA recognizes HIBCC as a UDI-issuing agency. HIBC-compliant barcodes meet FDA UDI labeling requirements and apply to all device classes under the UDI rule.

2. Global Trade Item Number (GTIN)

Purpose:
The GS1 system uses GTIN as a globally recognized identifier for trade items, providing a standardized approach to product identification across industries, including healthcare. GTIN supports UDI implementation and ensures consistency across international supply chains. Consequently, organizations use it to accurately match products between databases, even across organizational boundaries.

Formats:
Linear (GS1-128) and 2D (GS1 DataMatrix or GS1 QR Code). Encoded in various barcode symbologies, including UPC, EAN, ITF-14, and GS1-128, depending on the application.

Data Structure:
Numeric-only identifier, usually 14-digit product ID, with optional Application Identifiers (AIs) for Expiration date (AI 17), Batch/lot number (AI 10), Serial number (AI 21).

Usage:
Used globally for distribution of medical devices, pharmaceuticals, and consumables. In addition, it is widely used in supply chain automation, EDI, and healthcare procurement.

Regulatory Role:
GTINs play an integral role in Unique Device Identification (UDI) systems by facilitating product traceability and regulatory compliance. Manufacturers assign a unique GTIN to each type of trade item, recognizing the need to retrieve predefined information as the product or service moves through the supply chain. Companies may price, order, or invoice these items at any stage, whether they are individual units or various packaging configurations. Many countries require GTIN-based barcodes for their UDI programs, including the FDA (U.S.), EU MDR (Europe), NMPA (China), and Health Canada.

3. DataMatrix Barcode

Purpose:
A Data Matrix barcode is a two-dimensional (2D) barcode that stores data within a grid or matrix of black and white squares, also known as modules. It’s capable of holding a large amount of data in a small area, making it efficient for compact spaces. Manufacturers commonly use it to represent HIBC or GS1 UDI data on medical devices, aerospace components, pharmaceuticals, and food and beverage products for traceability and identification. 

Data Structure:
Comprises black and white “cells” arranged in a square or rectangular pattern.

High Data Density:
Can store up to 2,335 alphanumeric characters.

Error Correction:
Utilizes error correction methods, such as ECC200, which allows for some degree of damage or imperfections without compromising readability. 

Small Footprint:
Ideal for labeling small items and curved surfaces like surgical instruments.

Usage:
1) Marking small surgical instruments, implants, catheters, etc.
2) Direct part marking (DPM) for reusable devices and components.
3) FDA requires DataMatrix on many Class II and III devices.

Regulatory Role:
Regulators prefer DataMatrix as the standard format for 2D UDI labels. Additionally, the FDA requires it for reusable Class III devices. Both the FDA and EU MDR mandate direct marking with DataMatrix on small devices. Medical device manufacturers have adopted it for UDI compliance to support global harmonization and interoperability. Industries across healthcare, aerospace, and logistics use DataMatrix barcodes for inventory management, product tracking, and documentation.

 4. QR Code

Purpose:
A QR code, or quick-response code, is a 2D barcode that stores large amounts of data. It enables fast scanning and quick access to information. Specifically, it includes a locator, an identifier, and web-tracking data. As a result, smartphones can easily scan QR codes, making them especially popular in patient-facing applications. However, most regulated healthcare settings still do not use QR codes for UDI purposes.

Data Structure:
Consists of black squares on a white background with fiducial markers, arranged in a grid.

High Capacity:
Can encode up to 7,089 numeric or 4,296 alphanumeric characters. Since QR Codes carry information both horizontally and vertically, this code can encode the same amount of data in approximately one-tenth the space of a traditional bar code.

Error Correction:
Dirt and Damage Resistant. Includes error correction capabilities, allowing partial data recovery.

Versatility:
Readable from any direction in 360°. Commonly used in marketing and consumer applications such as product tracking, item identification, time tracking, document management, and general marketing.

Technology:
Easily scanned by smartphones and tablets, displaying the code and converting it to some useful form (such as a standard URL for a website). This thereby removes the need for a user to type it into a web browser.

Usage:
Linking to digital instructions for use (eIFU), patient education materials and post-market communication, such as recalls or product support.

Regulatory Role:
While the FDA does not recognize QR codes as a primary UDI carrier, they continue to play a growing regulatory role in various industries. They support compliance by facilitating adherence to regulations and standards. Additionally, QR codes enable businesses to share accurate, structured data with consumers and authorities. This helps streamline processes and improve transparency. Organizations also use QR codes for consumer advertising or in supplementary roles, such as patient-facing materials.

Recommendations

Medical Device Labeling: Adopt GS1 DataMatrix barcodes to ensure compliance with UDI regulations and facilitate global traceability.
Trade Item Identification: Utilize GTINs encoded in appropriate barcode formats (e.g., ITF-14 for logistics, GS1-128 for healthcare applications).
Hospital Inventory Management: Consider HIBC barcodes where applicable but be aware of the global shift towards GS1 standards.
Consumer Engagement: Use QR Codes for marketing purposes, but not for product identification in healthcare settings.
